Consent Form For 3 to 5 Years Old

Early intervention is a key to prevention. Alberta Education provides programming for eligible children in ECS who have a speech &/or language delay. Left untreated, speech/ language challenges can have a negative impact on school learning later on. Learning to read, write and focus are examples.

Regarding speech and language, parents sometimes believe, or are told by non-speech-language pathologists that their child “will grow out of it”. Most speech and language errors in the preschool years do correct themselves. But, if a delay exists, the longer the problem persists, the harder it is to treat. A speech-language screening/assessment should be considered if you and/or the teacher have questions about your child’s speech-language development. Some guidelines for speech-language development are attached. Therapists will contact you to discuss the results.

Please note: If your child is approved for support through Alberta Education we are required to provide access to 300 – 400 hours of early childhood programming from September thru June for children assessed with severe challenges, and 475 hours of early childhood programming for children assessed with mild-moderate challenges or who are English Language Learners. If your child attends a preschool or childcare program for less than 10 hours per week we may be able to offer additional programming which may take place in a group or one-on-one, and in a community setting or in home setting. Where sufficient additional programming is unavailable MaxECS may refer you to Alberta Health Services for support.

The service is provided at no charge to parents. If you would like a screening/assessment for your child, please complete the following, and return it on the day of the screening. A signed consent is required.
